In my experience, the source of most plems in an engineering company is under-communication. I believe it's vital to have technical management that can translate the needs and concerns of programmers to the rest of the organization, and likewise understand the constraints of the business, management, design, marketing, etc. in order to help all sides make the right choices for holistic success. In other words, people need to know "why" they are doing something.
